If you have insurance, we would have to determine whether the provider is within network for your plan, and if the type of service your are seeking is covered. We can perform a complimentary verification check, though it is recommended that you also check with your insurer to ensure consistency with the information we receive.

Please fax the written authorization from the claims’ adjuster for the service being requested to (866) 437-0375, along with patient demographics and available medical records. The following describe the verbiage that should be used in the authorization letter.
“Psychological Consultation” describes an initial evaluation with psychological testing for the purpose of diagnostic clarification or treatment planning. Pre-surgical psychological evaluations also qualify as consultations. Consultations may lead to a treatment relationship with the patient, should the results of the evaluation determine that the patient is an appropriate candidate for therapy. Opinions regarding causation and permanent disability will not be addressed if the doctor is a secondary treater.
“Neuropsychological Consultation” includes full cognitive and psychological testing for the purpose of diagnostic clarification or treatment planning. Consultations may lead to a treatment relationship with the patient, should the results of the evaluation determine that the patient is an appropriate candidate for therapy. Opinions regarding causation and permanent disability will not be addressed if the doctor is a secondary treater.
“Medical-Legal Neuropsychological Evaluation” if there purpose of the evaluation is to determine causation and impairment ratings for the claimant’s cognitive and/or psychological symptom. Since neuropsychology is no longer considered a QME subspecialty, requesting specific authorization for a “medical-legal neuropsychological evaluation” is recommended when med-legal opinions are needed.
“Psychological consultation + six sessions CBT” is recommended to expedite the process of approving therapy sessions. At PNBC, we require initial psychological evaluations of all patients prior to seeing them for therapy, to determine an accurate diagnosis and treatment plan, as well as assess whether the patient is an appropriate candidate for therapy.

You would then be asked to provide us with a copy of the FAA letter you received indicating the need for an evaluation. If we are able to perform the type of evaluation you need, you would then need to request that your medical records be sent directly from the FAA to our office. Pre-authorization is always needed for neuropsychological or psychological evaluations. For those, an order from a referring doctor may be needed. Some types of evaluations may not be considered “medically necessary” by insurance companies and would not be covered. Our biller may assist in verifying benefits and coverage for the services you are seeking.

Forensic cases require the evaluator and retaining attorney (or IME company) to enter into an agreement. We do not work on liens.
Please note that insurance companies, including Worker’s Compensation policies, do not pay for cancellation or no-shows. The time that is scheduled for patients is ensuring that the clinician and affiliated staff are dedicated to providing that patient with services.
